I know nothing of any recent incident, but I saw one go down a few years back. Three people in about an 18' DC outboard were trying to come in at the end of their trip and did it poorly. The tide was dropping and the mouth was churning pretty good. The pilot chickened out and wasn't maintaining speed, so a three footer broke over his stern and swamped them good. They just let go of the controls and the current shot them back out like the next eel in line to where everything on their boat died. Took maybe five to ten minutes for it to go completely under during which another small boat was able to recover the peoples. They probably could have kept it afloat if they had a good hand pump, but they would have needed a tow to some easier sanctuary to enter than the breachway or just beached it.

 

Seemed like a very easy mistake for someone to make if they weren't familiar with handling a boat in that sort of current. Wouldn't surprise me if something like that happened again.
